Includes general practitioners, specialists or surgeons.

2. - Alternative health care provider

Includes massage therapists, acupuncturists, homeopaths, naturopaths, Feldenkrais or Alexander teachers, relaxation therapists, biofeedback teachers, rolfers, herbalists, reflexologists, spiritual healers or religious healers.

3. - Dental professional

Includes dentists, orthodontists or dental hygienists.

4. - Telehealth service provider
