12 - High-frequency loss
High-frequency hearing loss is defined as an audiometric high-frequency (3000 Hz, 4000 Hz, 6000 Hz and 8000 Hz) pure-tone average greater than 15 dB in one or both ears based on the criteria from the American Speech-Language-Hearing Association (ASHA) guidelines for hearing loss thresholds.
