Services of independent health practitioners to evaluate and manage the health of patients in residential care facilities, by taking patient histories, making examinations, providing diagnoses, counselling patients, co-ordinating patient care, and writing prescriptions and other orders. Includes services of practitioners in group practice. Includes initial consultations for the purpose of determining a course of treatments.

Services of independent health practitioners to evaluate and manage the health of ambulatory patients, by taking patient histories, making examinations, providing diagnoses, counselling patients, co-ordinating patient care, and writing prescriptions and other orders. Ambulatory patients are those not admitted as hospital inpatients, or to residential care facilities, and care may be provided in such locations as the offices of health practitioners, at patients' homes, in community health centres, in emergency rooms, or hospital outpatient departments. Includes services of practitioners in group practice. Includes initial consultations for the purpose of determining a course of treatments.
