Guidelines for completing the List of Wholesale Operations
Purpose
This survey collects the financial and commodity information used to compile statistics on Canada's wholesaling industry.

This list will be used to update information on wholesale operations of your company included in your 2012 Annual Wholesale Trade Survey.
General guidelines
Information is pre-printed for each wholesale operation that, according to our records, was owned or operated by your business during the reporting period covered by the 2012 Annual Wholesale Trade Survey. Please verify the pre-printed information and correct as required.
If we have omitted any business wholesale operations or if you have acquired new operations not listed here, please report the name, address, and relevant details for each establishment. If you require more space, please attach a separate sheet.
Operations that your business acquired or disposed of in 2012 should be indicated in the column "Please provide date(s) for part year operation".
